Must be a 501(c)(3) Tax-exempt Organization Must be based and serve communities within the following Florida counties: Alachula, Hernando, Hillsbourough, Manatee, Marion, Pasco, Pinellas, Polk, Sarasota Have a track record of quality service delivery and demonstrated results Demonstrated fiscal responsibility and long term sustainability Request form must be completed and submitted online by November 1, 2017 to be considered for the calendar year 2018.
